Many people in the UK are confused about which asbestos they can remove themselves and when they need to use a specialised contractor. Before I attempt to explain the whys and whens I’d just like to expand upon the word “asbestos” because its rarely used in the correct context and by-and-large it determines what you can and can’t remove.

Asbestos is a generic term for a family of fibrous minerals. When people say that something is asbestos they normally mean “it contains asbestos fibres”. The most commonly found members of the asbestos family are Crocidolite (aka blue asbestos), Amosite (brown) and Chrysotile (white). Different asbestos types have different properties so they tended to be used in different materials. For example white asbestos has long, strong and flexible fibres and is commonly found in construction materials such as roof sheets, paints and tiles. On the flip side, brown and blue asbestos have strong, fire-resistant (but brittle) fibres and tend to be used in thermal insulation products such as pipe laggings, sprays and boards.

Now that we’ve established the asbestos types its important to make a distinction between domestic and non-domestic work. Asbestos is regulated in the UK by the Control of Asbestos Regulations which is aimed specifically at the workplace (i.e non-domestic) and does not cover asbestos in the home. So, effectively, you are free to do what you want with asbestos in your own home. But, its important to remember that if, at any point, you hire someone in your home it becomes a workplace. Finally, before commencing any removal work in your home you need to consider how you are going to dispose of the waste. Many local authorities will accept white asbestos materials (roof sheets, tiles, Artex, etc) as long as they are correctly bagged. For all other products (AIB, sprays, insulations, etc) you will need to use a licensed waste carrier and you may find the cost prohibitive.

Right, asbestos removal in the workplace. The official way of working out when you need to use a licensed contractor is VERY complex and for the layman its almost impossible to calculate with 100% accuracy. Its based on the concentrations of fibres that you will be exposed to during the removal process. So, clearly, removing materials that contain high levels of asbestos fibres will expose you to greater numbers of fibres, right? True, but you also need to take account of factors such as whether its in or outside, which method you are using to remove it, and so on. Believe me its not simple. But, as a rule of thumb we can base it on material type and the following is 95% accurate.

Materials which can removed by a non-licensed person or organisation;

Composite materials (mostly white asbestos) i.e

- Asbestos cement (profiled roof sheets, panels, guttering, water tanks, etc)

- External tiles (roof or patio)

- Textured Coating (Artex)

- Internal plastic tiles and linoleoum

- Felts and bitumens

- Toilet cisterns (brown asbestos)

Materials which should be removed by a licensed contractor

Friable materials (normally blue and brown asbestos) i.e

- Asbestos insulation board (cladding, fire breaks, ceiling tiles, soffits)

- Sprayed insulation

- Packing

- Thermal insulations (pipe laggings)

Its important to bear in mind that any asbestos removal or repair works can cause significant asbestos exposure in both the long and short term and you should always consider using a specialist contractor.

Mesothelioma is a deadly disease that has taken thousands of lives.  It is caused by the inhalation of asbestos dust.  Although no cure has been found, diligent researchers continue to study the disease and its relationship to asbestos exposure.  One interesting study is called, “Asbestos causes apoptosis in alveolar epithelial cells: Role of iron-induced free radicals” – Journal of Laboratory and Clinical Medicine, Volume 137, Issue 5, Pages 330-339.  Here is an excerpt: “Asbestos causes asbestosis and malignancies by mechanisms that are not fully understood. Alveolar epithelial cell (AEC) injury by iron-induced reactive oxygen species (ROS) is one important mechanism. To determine whether asbestos causes apoptosis in AECs, we exposed WI-26 (human type I–like cells), A549 (human type II–like cells), and rat alveolar type II cells to amosite asbestos and assessed apoptosis by terminal deoxynucleotidyl transferase–mediated deoxyuridine-5′-triphosphate-biotin nick end labeling (TUNEL) staining, nuclear morphology, annexin V staining, DNA nucleosome formation, and caspase 3 activation. In contrast to control medium and TiO2, amosite asbestos and H2O2 each caused AEC apoptosis. A role for iron-catalyzed ROS was suggested by the finding that asbestos-induced AEC apoptosis and caspase 3 activation were each attenuated by either an iron chelator (phytic acid and deferoxamine) or a OH scavenger (dimethyl-thiourea, salicylate, and sodium benzoate) but not by iron-loaded phytic acid.

To determine whether asbestos causes apoptosis in vivo, rats received a single intratracheal instillation of amosite (5 mg) or normal saline solution, and apoptosis in epithelial cells in the bronchoalveolar duct regions was assessed by TUNEL staining. One week after exposure, amosite asbestos caused a 3-fold increase in the percentage of apoptotic cells in the bronchoalveolar duct regions as compared with control (control, 2.1% ± 0.35%; asbestos, 7.61% ± 0.15%; N = 3). However, by 4 weeks the number of apoptotic cells was similar to control. We conclude that asbestos-induced pulmonary toxicity may partly be caused by apoptosis in the lung epithelium that is mediated by iron-catalyzed ROS and caspase 3 activation.”

Another interesting study is called, “Comparative Proliferative and Histopathologic Changes in Rat Lungs after Inhalation of Chrysotile or Crocidolite Asbestos” by
Kelly A. BéruBéa, Timothy R. Quinlana, Gerald Moultona, David Hemenwayb, Patrick O’Shaughnessyb, Pamela Vacekc and Brooke T. Mossmana – Toxicology and Applied Pharmacology – Volume 137, Issue 1, March 1996, Pages 67-74.  Here is an excerpt:  “Patterns of cell proliferation in lung and pleura and development of histopathologic lesions were studied in lungs from Fischer 344 rats after inhalation exposure to chrysotile or crocidolite asbestos at average airborne concentrations of approximately 8 mg/m3air for 5 and 20 days and after 20 days of exposure followed by an additional 20 days in room air (20 + 20 days). To assess cell proliferation rats were injected with 5-bromo-2′-deoxyuridine (BrdU) at various time points after initiation of exposure to asbestos. Image analysis was used to quantitate the effects of chrysotile and crocidolite on BrdU labeling indices in the following lung compartments: (1) interstitium, (2) alveolar duct region, (3) bronchial epithelium, and (4) visceral mesothelium. With the exception of mesothelium, which exhibited significant increases in BrdU incorporation in rats exposed to crocidolite at 20 + 20 days, asbestos-induced elevations in BrdU uptake in other compartments were transient with labeling comparable to sham controls at later time points.

Histopathology of rat lungs revealed fibrotic lesions of a greater extent and severity at 20 days in rats exposed to crocidolite, but fibrosis occurred in both asbestos-exposed groups after an additional 20 days in clean air (20 + 20). Quantification of fiber burden in rat lung after inhalation of comparable airborne concentrations of either fiber type demonstrated that inhalation of crocidolite asbestos led to a higher fiber retention when compared to chrysotile asbestos. Our results indicate that chrysotile and crocidolite asbestos induce different patterns of cell proliferation in lung and pleural cells. The protracted increases in BrdU labeling of mesothelial cells by crocidolite may reflect increased retention of fibers and/or inherent differences between types of asbestos.”

Mortality and death rates increase as cumulative exposure to asbestos increases.  One group of employees that have been closely studied are those in the railroad industry.

One interesting study is called, “Mortality among asbestos-exposed workers in a railroad workshop.” By Ohlson CG, Klaesson B, and Hogstedt C. – Scand J Work Environ Health. 1984 Oct;10(5):283-91.  Here is an excerpt: “Abstract – The mortality experience of a cohort of 3 297 railroad maintenance shopworkers exposed to asbestos was investigated. The study period was 1951-1980, and the vital status was assessed for 99.6% of the men. Individual estimates of cumulative asbestos exposure were based on detailed records on work tasks and divisions. Dust measurements were scanty in earlier decades, and estimates of average fiber levels were therefore based on information on the amount and kind of asbestos used, job descriptions, and interviews with older workers. The overall mortality was lower than expected from the national death rates (standardized mortality ratio = 82). The mortality from lung cancer increased as cumulative exposure increased in consistent dose-response relationships. Employment times of less than 30 years in workplaces with moderate levels of mainly chrysotile asbestos was not associated with any apparent increase in the risk of lung cancer. A subgroup exposed for more than 30 years in workplaces repairing steam engines, where amphiboles were used as well, had a standardized mortality ratio of 192 for lung cancer. This figure may be an underestimation due to healthy worker selection and fewer smokers than normal. The “true” standardized mortality ratio was estimated to be about 300. Five cases of mesothelioma were observed.”

A second study that is interesting is called, “Cellular ingestion, toxic effects, and lesions observed in human bronchial epithelial tissue and cells cultured with asbestos and glass fibers” by Aage Haugen, Paul W. Schafer, John F. Lechner, Gary D. Stoner, Benjamin F. Trump, Curtis C. Harris – Radiation Oncology Investigations – Volume 30 Issue 3, Pages 265 – 272.  Here is an excerpt: “Abstract – Although exposure to asbestos has been shown in previous epidemiological studies to synergistically increase the risk of bronchogenic carcinoma for cigarette smokers, the pathobiological mechanism(s) responsible for the cocarcinogenic action of asbestos is not known. Therefore, the effects of asbestos fibers on normal human tracheobronchial epithelium are of interest. Asbestos fibers, i.e., amosite, crocidolite and chrysotile (UICC samples) and glass fibers were initially assayed for their cytotoxcity. Bronchial epithelial cells exposed to either asbestos or glass fibers displayed inhibition of cell growth as a function of fiber concentration. When compared to glass fibers, asbestos fibers caused a statistically significant (p<0.05) decrease in cell population doubling rate. Chrysotile was approximately 10 times more cytotoxic than either amosite or crocidolite and more than 100 times more cytotoxic than the glass fibers. Epithelial cells were 10 to 15 times more sensitive to the cytotoxic effects of asbestos fibers than bronchial fibroblasts from the same donor. Using electron microscopy, asbestos fibers were seen to be ingested by epithelial cells. After exposure for 2 h short fibers (<12m) were found both in the cytoplasm and within phagosomes. High-voltage electron microscopy combined with stereo-microscopy revealed asbestos fibers in the cytoplasmic matrix.

A single exposure to amosite asbestos (100-1000 g/ml) was found to induce focal hyperplasia and epidermoid metaplasia with cellular atypia in human tracheobronchial explants. Surface fine structure observations revealed asbestos fibers protruding from these lesions. Intra-cytoplasmic and intranuclear fibers of amosite were identified in these focal lesions using X-ray microanalysis in combination with transmission electron microscopy. With increasing time, fibers were also seen deposited within the submucosa of the explants. The preferential cytotoxicity of asbestos for epithelial cells and the metaplastic response of bronchial epithelium may be important in the cocarcinogenic action of asbestos. “

A third study is called, “Pleural calcification associated with exposure to chrysotile asbestos in southeast Turkey.” By Yazicioglu S. – Chest. 1976 Jul;70(1):43-7.  Here is an excerpt: “Abstract – In this study, pleural calcifications prevailing in the inhabitants of some villages and towns around Diyarbakir, a city in southeast Turkey, have been found to be caused by the inhalation of chrysotile asbestos, the mineral commonly used indiscriminately to paint the walls and floors of houses. There were no industrial establishments in the area, except some local mining, and asbestos deposits were even not included on the mining map of the country. Chest roentgenograms from the Chest Hospital and the tuberculosis dispensaries were screened: and, additionally, 15,239 photofluorograms were obtained during a survey. Of all these, 511 patients were found to have pleural calcifications, some with pericardial and diaphragmatic involvement. Forty-four percent (28) of the 64 patients who had examinations of samples of their sputa had asbestos bodies, and asbestosis was also induced in rabbits by administration of the raw material.”

Most people are exposed to asbestos in the workplace, often due to lack of proper safety procedures and shortcuts.  Some occupations are considered more dangerous than others.  One interesting study is called, “A mortality study among workers in an English asbestos factory” By J Peto, R Doll, S V Howard, L J Kinlen, and H C Lewinsohn – Br J Ind Med 1977;34:169-173.  Here is an excerpt: “Abstract – The previous report on this cohort study of workers in an asbestos textile factory (Knox et al., 1968) showed little evidence of increased mortality among workers who had entered the factory after the implementation in 1932 of the first Asbestos Industry Regulation (1931) but observed that no firm conclusions could be drawn, as little carcinogenic effect would be expected for 20 years after first exposure. A further 8 1/2 years of follow-up has revealed some asbestos-related disease in this latter group, although very much less than for employees first exposed before 1933. Among the 963 workers first exposed in 1933 or later, mortality was increased for carcinoma of the bronchus (31 deaths; 19-3 expected for all lung cancers) and non-malignant respiratory disease (35 deaths, 25-0 expected), and a further 5 deaths were attributed to pleural mesothelioma.”

Another interesting study is called, “Effects of cigarette smoke and asbestos on airway, vascular and mesothelial cell proliferation.” By Sekhon H, Wright J, Churg A. –
Department of Pathology, University of British Columbia, Canada – Int J Exp Pathol. 1995 Dec;76(6):411-8.  Here is an excerpt: “Abstract – In order to determine whether exposure to both cigarette smoke and asbestos leads to enhanced cell proliferation, and whether pleura cell proliferation reflects the presence of fibres at or near the pleura, rats were exposed to air (control), daily cigarette smoke, a single intratracheal instillation of amosite asbestos, or a combination of smoke and asbestos. Dividing cells were labelled with bromodeoxyuridine (BrdU) and animals were sacrificed at 1, 2, 7 or 14 days. Both cigarette smoke and asbestos produced increases in the labelling index of small airway wall, epithelial cells and pulmonary artery cells. In the small airways there was a brief marked positive synergistic interaction between these two agents, but synergism was not seen in the vessels. Cigarette smoke did not increase the labelling of mesothelial or submesothelial cells, whereas asbestos caused a persisting increase in mesothelial cell labelling. There was no correlation between the number of BrdU labelled mesothelial or submesothelial cells and the number of fibres touching the pleura, or located within 180 microns of the pleura. We conclude that the combination of cigarette smoke and asbestos exposure produces a complex set of interactions and has the potential to markedly increase cell proliferation in the parenchyma, an effect that may be important in both fibrogenesis and carcinogenesis. In contrast to the diminishing effects over time of a single dose of asbestos on cell proliferation in the small airways and vessels, the same dose of asbestos leads to sustained mesothelial cell proliferation. However, the latter process does not correlate with local accumulation of asbestos fibres.”

A third study is called, “Asbestos bodies, their formation, composition and character” by F. D. Pooley – Environmental Research – Volume 5, Issue 4, December 1972, Pages 363-379.  Here is an excerpt: “Abstract – Asbestos bodies extracted from human lungs have been examined under the electron microscope to determine their morphology and diffraction characteristics. The asbestos bodies were extracted from the lungs of persons exposed to the four types of commercial asbestos fiber, namely cocidolite, amosite, anthophyllite and chrysotile.

Asbestos bodies were rarely formed on fibers less than 10 μm in length and practically always on straight fibers. They were found to occur on all the four commercial varieties of asbestos but less frequently on chrysotile fibers. The characteristics of the individual fibers were found to play a large part in determining the range and frequency of body shapes observed. There appears to be no evidence to suggest stepwise formation of asbestos bodies.

Asbestos bodies were found to have a major crystalline component which is structurally similar to extracts of ferritin produced from animal and human organs. The material forming the asbestos body is definitely biological in origin and corresponded closely to the material defined in the medical literature as hemosiderin.”
